Job Description
We are seeking an experienced RN Case Manager for a large Medical Group located in the Manteca/Modesto area.
The Case Manager has the responsibility to identify, plan, coordinate, implement, monitor and evaluate appropriate cost-effective health care services for patients of the Medical Group in order to optimize patient function and well being. This position provides for optimal outcomes through collaboration with the client, the family, the physician and other members of the healthcare team.
REPORTING RELATIONSHIPS:
The Nurse Case Manager reports directly to the Manager of Care Management
MINIMUM POSITION REQUIREMENTS:
Education:
· Current California RN licensure.
· BSN/PHN preferred.
· National certification in Case Management preferred.
Experience:
Experience developing care plans.
Experience working as a member of a multi-disciplinary team.
3 years’ experience in case management, knowledge of health-related information, education, resource and referral services or 5 years general clinical experience.
