We are seeking a Director of Nursing for Wagner Heights Nursing and Rehabilitation Center in Stockton, CA. We have 152 beds and are a fast paced environment. As a result, we are looking for someone who is competent clinically, a strong leader, focused on quality and able to maintain strong communication with our referring providers. The primary responsibility of this role is to lead and manage the Nursing Department and utilize available resources to provide quality, caring services to all residents and patients, and achieve desired clinical and financial results. This role will be heavily focused on quality, education, maintaining our solid infrastructure, leadership and deepening out already strong staff morale. The goals for this role would include but not limited to:Achieve company and facility clinical indicatorsAchieve facility goals for annual state survey ensure nursing programs are provided for the facility (in:services)Facilitate and sustain effective IDT (Interdisciplinary Team) walking rounds per protocolPromote clinical and revenue improvements through effective participation in the PPS care management processPromote clinical and revenue improvements through Part B and other services (therapy)Effectively manage nursing hours PPD (Per Patient Day) compared to appropriate budget for actual census and mix (type of payer source for census) and state requirements. BUILDING DETAILS:5 STAR in Quality Measures. 152 beds facilitya go to facilities for patients needing specialized care in: Cardiac Recovery, Orthopedic and Wound care. 1 of 2 west coast facilities using Synchrony, an advanced Speech Modality, to successfully treat complex Stroke Patients, as well as Pneumonia patients.Very strong clinical outcomes and as a result they have significantly reduced our re:hospitalization rates. Experience to include a minimum of two (2) years as a supervisor in a hospital, long:term care facility, or other related health care facility.Experience to include a minimum of two (2) to three (3) years training experience in rehabilitative and restorative nursing practices.Current, unencumbered, active RN license in the working state.Must have an active and unrestricted California Registered Nursing LicenseBachelors degree is highly preferred.
